I have custody of my two boys 5 and 4 years old. My ex wife has been diagnosed with Borderline Personality Disorder, her grandmother said she has been cutting herself, there has been several episodes over the last year where she has not picked them up for her scheduled visitation, or has picked them up and had a mental break down. Once she had them for the weekend and her boyfriend was there. She could not get them to go to bed so she could go out dancing for the evening, so she broke glasses, locked herself in the bathroom and took a bunch of sleeping pills. All while the kids were there. She recently had DHS show up at our door because she accused me of abusing my family. Their report came back with nothing of course. But she has tried everything including telling the boys that she is saving up money so they can live with her. How do I get supervised visitation with this information? I'm scared she will put them in danger or kidnap them. How do I do it without an attorney? Or on a low income? I live in Oregon.

   You report her to DHS.  Keep in mind that they will be slow to respond, as they will think it is in retaliation for her reporting you.

 Gosh, you have indeed an awful problem there and must be out of your mind with worry about your kids when they go on visits with their mother.I do not know how you could file for supervised visitation - perhaps you could simply tell the DHS about the boys' mothers mental health problems. Borderline Personality Disorder, if that is what she indeed has, is a horrible problem, mostly because the people with it are resistant to treatment of any kind. Even if they are accepting of treatment it is unlikely to work. Actually in Seattle there is a lady Psychiatrist who is known to be a world wide expert on the problem. Tons of hours have to be put in however, and not too many therapists are available or willing for that.Failing the supervised access, I would tell the boys to call you on the phone if they have any trouble at all with their Mom, or call someone. Perhaps there is a neutral party, Grandmother?  who could help.Trouble is if you start causing trouble for your ex wife, the powers that be see it as a disaffected parent trying to blame the other. This happens in custody battles all the time.You are very fortunate to have custody, and the boys are very lucky to have you.
